Who we are

DailyMeal Care is an independent community outreach service. We are not a government agency and we are not an official New York State Social Care Network. We help eligible New York Medicaid members connect to free Social Care Network services through their plan.

Not an application; no guarantee of services

This website is not an application for government benefits. Submitting the eligibility check does not enroll you in any program and does not guarantee that you will receive meals or other services. Final eligibility and the availability of any services are determined by your Medicaid plan and its contracted Social Care Network partner.

No impact on your Medicaid benefits

Using this website to check your eligibility is free. Participation will not affect your Medicaid benefits.

Information you provide
