DailyMeal Care ("we", "us", "our") is an independent community outreach service that helps eligible New York Medicaid members connect to free food and nutrition services through their plan's Social Care Network benefits. This Privacy Policy explains, in plain language, what information we collect, how we use it, and who we share it with.

Information we collect

• Contact details you provide: name, phone number, ZIP code, and (if you choose) your Medicaid ID.
• Your responses to the eligibility questions on this site, including general health-related answers you choose to share.
• Basic technical information your browser sends automatically (such as date and time of submission).

Providing a Medicaid ID is optional. You may complete the eligibility check without it.

How we use your information

We use the information you provide to:

• Contact you about whether you may qualify for Social Care Network services.
• Share your information with a contracted Social Care Network partner so they can coordinate services on your behalf.
• Keep internal records of submissions so we can follow up and improve our outreach.

We do not sell your information and we do not use it for third-party marketing.

Who we share information with

We share the information you submit only with the contracted Social Care Network partner needed to coordinate the services you may be eligible for, and with service providers that help us operate this website (for example, hosting and form storage). These providers are only permitted to use your information to support our operations.

Protected health information & HIPAA

Some of the information you choose to share may be considered protected health information ("PHI"). We follow the requirements of the HIPAA Privacy Rule when handling any PHI you provide and limit access to staff and partners who need it to coordinate your services.

Your choices

• You can ask us to delete the information you submitted.
• You can withdraw your consent to be contacted at any time.
• You can ask us what information we have about you and request corrections.

Data retention & security

We keep your information only as long as needed to coordinate services and meet our recordkeeping needs. We use reasonable administrative and technical safeguards to protect it. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

Children

This website is intended for adults. We do not knowingly collect information directly from children.
